Implementing incident management can be a difficult and drawn out process when you have to consider: tools, management, work-flows, access rights, severity & priority definitions etc... Sometimes this can take months before all parties involved agree on the appropriate action.
So what if your team takes ownership of a live product before your incident management process is in place? What do you do if someone discovers a serious live defect?
